Changing Information on Your Driver’s License

You can apply to amend information on your driver’s license at the Driver’s License Center or at police station (Application by proxy is also acceptable).

Requirements

Applicants will need:

• Driver's license
• An application form for changing information on a driver’s license.
  (You can get the form both at the Driver's License Center and at your local police station.)
• A Certificate of Residence that shows your legal domicile, or nationality (foreign applicants only) if you have any changes to your name, date of birth, legal domicile, or nationality.
  * You can apply only at the Driver’s License Center if you have a change to your date of birth.
• A document to confirm your new address such as a Certificate of Residence, a Resident Card, a Health Insurance Card, ID card, or postal matter delivered to your new address, if you have a change to your address.
  * You will need to provide an original of Certificate of Residence. No photocopy is acceptable. Your Certificate of Residence will not be returnable.

Procedure

1. Filling out an application form

2. Application
   受付
3. Writing new information on the reverse side of your license
   運転免許証の裏面に変更事項を記載
4. Returning your driver’s license
   運転免許証を返却

Note: If you apply to amend information on your driver’s license by proxy, your proxy also must bring ID. A letter of proxy will not be required.
